I want to join the BAUEN DENTAL Oral Health Plan, how should I proceed?
You can join the BAUEN DENTAL Health Plan through our website in the tab JOINBy filling in the form and submitting your Membership Application Form, or alternatively you can apply through any of our sales agents.
What benefits do I have with the BAUEN DENTAL Oral Health Plan?
The BAUEN DENTAL Oral Health Plan has no waiting period or age limit. It also allows you to benefit from a very accessible price table, thus being able to take care of your Oral Health, besides including various treatments at zero cost.
Is there a binding contract?
Yes. The contract has a 12-month loyalty period.
How long does the contract last?
The contract has a minimum duration of 12 months and is automatically renewable if no opposition to its renewal is made 15 days prior to the expiry date, or its renewal.
What are the procedures for inclusion and exclusion of beneficiaries?
In case you want to include or exclude beneficiaries to your contract you should take into consideration the following:
InclusionsTo include new beneficiary(ies) to your contract you must fill out a new Subscription Application Form and include the beneficiary(ies) you want. You should take into account that whenever you include a new beneficiary to your contract, the 12 months of contract loyalty period restarts from the date of the inclusion.
ExclusionsTo exclude beneficiary(ies) from your contract, you must respect the contractual deadlines. Beneficiaries can only be excluded at the end of the contract by written request 15 days before the end of the contract or its renewal.
How can I update my personal data?
You must send an e-mail to: alteracoes.plano@bauendental.pt requesting/informing of changes that have occurred.
Can I cancel the contract by phone?
No. The cancellation request can only be made in writing (mail or letter) to the addresses indicated in the contract.
How to proceed to cancel the contract?
To cancel the contract, you must send your cancellation request in writing, e-mail or letter to the addresses listed in clause 3.4 of the contract, indicating your customer number, or nif, requesting the cancellation/opposition to renewal, 15 days before the expiration date of the contract or its renewal.
